Health Forms & Medical Requirements

To ensure the safety and well-being of all campers, Benchmark Hockey Camps requires the following health documentation to be submitted prior to camp.

Required Forms

1. Most Recent Physical

• Please provide a copy of your child’s most recent physical.
• The physical must have been completed within the past two (2) years.

2. Authorization for Administration of Prescription Medication

• This form is required only if your child has prescription medication.
• This includes (but is not limited to):
• EpiPens
• Inhalers
• Any other prescribed medication
• All prescription medications must be properly labeled and submitted with this authorization.

3. Over-the-Counter (OTC) Medication Authorization

• This form gives our staff permission to administer common over-the-counter medications if needed.
• Examples include:
• Tylenol
• Ibuprofen
• Benadryl
• Sunscreen
• Similar OTC items

How to Submit Forms

• Forms may be uploaded directly to your camper’s account, or
• Sent via email to camps@benchmarkhockey.com

Important Note

Campers are not required to submit health documents at the time of registration, but all forms must be completed and received prior to camp arrival.
